Our post hospital support service exists to bridge that gap. Offering calm, consistent, human support when people are at their most vulnerable.

Why Important
Due to the lack of NHS funding and limited community resources, many people are discharged home with minimal follow up support. Many undergoing the most invasive treatment with not a big network around them to support the general day to day running whilst they can solely focus on getting through it. Yet this is often when individuals need help most.
We provide gentle, non-clinical support that helps people feel safe, seen and supported, without rushing recovery or placing pressure on independence.
